What is a Patient Centered Medical Home?
The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
The Patient Portal is a new way to review your medical records from the internet. The portal is a secured website that employs password authentication and other security measures to provide safety and confidentiality during login and throughout the session.
In order to confirm your identity and protect your medical records and other private information, we require patients to request their portal access ID & password, in-person, at their Skylands Medical office. No appointment necessary, just identify yourself to the reception staff and they will quickly activate your account and provide your username & password to the Portal system.
